Dental Insurance

Good oral and dental hygiene is important to keep you smiling. Foundations offers a great plan with Guardian.

$2,500 a year in coverage, so you can get what you need, when you need it without hitting that ceiling right away.
Regular check-ups and cleanings every 6 months
Your kids are eligible for orthodontics care under your plan (up to a $2,500 maximum).

When am I eligible for benefits with Foundations?

You become benefits eligible after your 30th day of consecutive service with us as a full time team member.

What happens if I don’t enroll in benefits during my onboarding?

No worries, you’ll simply have to wait for our special Open Enrollment (OE) period which happens every November. If you have a special life changing event, like a marriage or you lose coverage elsewhere, talk with us about it and see if we can help!

What is the team member introductory period?

Its the period of time that happens between when you are hired and when you are eligible for benefits. You are eligible as a full time team member after day 31. Your benefits will begin the first of the calendar month following this date.

Am I required to take benefits from Foundations?

Absolutely not, all benefits programs with Foundations are optional. However, to be enrolled in no cost or low cost benefits with us, you must enroll in them during your onboarding or during an open enrollment window.

How much do benefits cost?

Our benefits are rated individually based on age and other criteria that our insurance companies use to determine cost and eligibility, such as medical. Other benefits are flat rate and based on age.
